DOCUMENTS NEEDED FOR THE PROCESSING OF YOUR CONTRACT OF LEASE:
 DTI Registration
 4 copies of location Plan Annex “B” signed by Lessee
 Government Issued ID with a clear specimen signature (photocopy)
 Certificate of Registration (BIR Form 2303), Address should be the same in the Leased Premises
 Authority to Print (BIR Form 1921)
 BIR Permit to use POS (if applicable)
 Franchise Agreement (if applicable)
Note: Failure to submit all of the above documents will result delay of the notarization of your Contract of Lease

1 Trade Name should be the same in the DTI Registration
2 Complete name should be the same with the Government Issued ID
3 For Married, Spouse's name is required.
